Add questions and answer options

Choose a question type based on the answer you need. Add questions inside a page in Content; use the type selector in the question header to choose its type.

Type Purpose Example
Single choice Ask for one option. “Which workshop did you attend?”
Multiple choice Allow several options. “Which topics would you like next?”
Ordinal Ask for one value on an ordered numeric scale. “How strongly do you agree, from 1 to 5?”
Text Collect a written answer. “What should we change?”
Information Show instructions or other content without collecting an answer. “Please answer about today's workshop.”

Write the question in Question prompt, or instructions in Information text. For Single choice and Multiple choice, use Add option and write each answer label. To explain an option in more detail, enable survey Settings → Show tooltips, then edit its Tooltip field in Content. For Text, Placeholder supplies a hint in the answer field; it is not a saved answer.

Configure an ordinal scale

Choose Ordinal in the question header, write the prompt, then set the Range:

Range Values to enter Example
1 → n The last value. 1, 2, 3, 4, 5
0 → n The last value, including zero at the start. 0, 1, 2, 3, 4, 5
Range The first and last values, including negative numbers. -5 to -1, or -3 to 3
Custom Distinct whole numbers in ascending order. -2, 0, 1, 3, 7

Use whole numbers between -9007199254740991 and 9007199254740991, with at most 1,000 values in one scale.

Under Buttons & subtitles, each value has its own optional Subtitle field. For a five-point agreement scale, you might label 1 as Strongly disagree, 3 as Neutral, and 5 as Strongly agree. Leave any other subtitles empty. Participants choose one button from a horizontal row. The subtitle stays with its number wherever that button appears.

Use the separate Preview pane to check the answer buttons. The question card edits the scale; it does not contain an embedded answer preview.

Randomize ordinal direction

Enable Randomize ordinal direction in survey Settings to vary which end of the scale appears on the left. The same toggle appears in each ordinal question's settings. Both controls change the survey-wide setting; changing it from one question affects all ordinal questions in the survey.

The setting starts off, so values appear from lowest to highest. When enabled, some participants see lower values on the left and others see higher values on the left. Each participant keeps the same direction across all ordinal questions, including when going Back, reloading, or resuming their response. This reverses the whole scale; it does not shuffle individual values.

Reversal changes presentation only. Selecting 5 records the value 5 in either direction, and exported answers keep that numeric value. Subtitles do not replace the saved value.

Require an answer

Open Settings · validation, privacy & order and enable Required. Participants must answer that question before continuing. Leave it off when skipping the question should be allowed.

For a long written answer, enable Allow multi-line answers and set Lines shown by default. That setting controls the initial field height, not the answer length.

Example: require the workshop rating, but leave the written comments optional. This collects the rating without forcing participants to invent a comment.

Mark identifying answers

Enable Is PII for answers that can identify someone, such as their name or email address. PII means personally identifiable information. Review the survey's retention settings before publication; marking an answer does not replace choosing an appropriate deletion schedule.

Page and group settings also provide bulk controls for required answers and PII. Check the affected questions before applying a bulk change.

Keep options clear

Use labels that participants can distinguish without reading a tooltip. For example, use Every day, Several times a week, and Less often rather than unexplained codes.

Single choice and Multiple choice settings control option order and how many options appear. Ordinal scales use the survey-wide direction setting described above. See Randomize content. When you reuse a question, synchronized templates can keep its wording and settings consistent.

Question IDs and scripts

Question IDs are readable references for conditions and exported results. Enable Show IDs in survey Settings to display these fields in Content. Use a recognizable, unique ID when you need to refer to a question elsewhere. A choice option also has its own ID. Use the same Question ID to read its answer in JavaScript.

For custom browser behavior, expand the question settings and use its JavaScript controls. See question scripts before adding code.

To add a question that stays in the DOM without appearing on the participant's page, use hidden questions. Their settings are at the bottom of the question's Settings panel.
